Dr. Dwyer and Dr. Leschek have been practicing neurology in Frederick for over 20 years.  They have earned the trust of the community and have a strong reputation among the area's local physicians.  Patients are given the time and personal attention they need to diagnose and treat a wide variety of neurological conditions.  You will be working directly with your doctor at each visit.  We pride ourselves in our punctuality.  We know you value your time as much as we do. New Patients
New Patients can download the New Patient Packet. All new patients must complete a New Patient History, Patient Registration and read and sign the Office Policy and HIPAA Notice of Privacy Practices  before their visit. Please arrive 10 min early and have these forms completed. If you are late, do not have your paperwork completed, or don't have a proper referral, you may be asked to be rescheduled.  All forms are all included in the New Patient Packet download. We require you bring all pertinent insurance and medical information to your first visit. This includes medical reports, labs and especially MRI or CT films or discs. We prefer to see the actual brain or spine CT or MRIs.

EMG patients: DO NOT
use oils or lotions on the arms or legs to tested! This will  degrade the results of the test and may result in our asking you to reschedule your test.

New and Follow-Up Patients
*Remember to bring actual copies of your new MRI or CT films.*
We always want to look at new studies.
Many imaging centers will not automatically send us the films.
